On Tuesday February 6, 2018, Cook was seen by witnesses go into the water at Huntington Beach Pier and not come back. Her disappearance was reported to the Coast Guard at around 11:40 p.m. local time. ^{[4] [1] [6]}

Some of her personal items were also left behind at the pier.

Her body was found in the ocean near Tower 7 Thursday afternoon on February 8, 2018.

A spokesperson from the Orange County Sheriff's Department (California) had positively identified her. ^{[4] [1] [6]}
