On September 22, 2016, Swafford shot and killed two of his supervisors before turning the gun on himself inside the Thomas & Betts Corp. plant in Athens, Tennessee. ^{[1] [undefined]}

Swafford became visibly upset and burst out from a meeting with James A. Zotter, 44, and Sandra H. Cooley, 68 around 4:15 p.m. ^{[1] [undefined]}

He then returned a short time later with a Semi-automatic pistol and fatally shot both Zotter and Cooley. As responding officers rushed over to the plant, Swafford walked into a bathroom were he reportedly shot himself. ^{[1] [undefined]}
