Services
Evolent Health begins each endeavour with a consulting engagement, called the Blueprint, which allows their team to assess the current state of the health system and provide further recommendations on how to start the path toward a value-based health system.
[1] Evolent is focused on the prevention of future illness and how to improve the efficiency of health systems.
[1] Through their technology platform, Identifi, [1] data analytics enable physicians to identify high risk individuals and create targeted interventions personalised to that individual. Care is coordinated through a care team that works to engage the patient in their health and provide all aspects of care and assistance to the patient as needed. [1]
By offering consultation, implementation and even having a few employees embedded on site, Evolent is able to form long-lasting partnerships and work with the system throughout the whole transition, acting as a coach by offering advice and expertise.
